keyboardstethNew Patient Registration

The doctors welcome new patients who live within our practice area.  Please call into the Surgery to complete a Registration Form, or alternatively bring along your NHS Medical Card. You will also need to complete a Medical Questionnaire and will be offered a New Patient Check with the Practice Nurse.

All patients are currently being registered with Dr Bloemertz for administration purposes but you can choose to visit any of the GPs you wish.  Once registered, your medical records will be automatically transferred from your previous surgery. Medical treatment is available from the date of registration.  Please contact Reception for further information and a Practice Leaflet.

Regsitration Forms

You may print off a registration form & medical questionnaire, fill them out and bring them in with you on your first visit to the practice. Both documents are required for registration.
